Practical things, in the order you are likely to need them.
What to bring:
For a child’s first visit, also useful: growth records if you have them, the name of any specialist they see, and a school or child care form if one is due — asking for it at the same visit saves a second trip.
If you are new to Arizona, worth mentioning: where you moved from and when. It changes what is worth screening for, and it is the sort of thing nobody thinks to say.
Someone takes your height, weight and blood pressure, and asks what brought you in. Then the clinical part, then a plan you should leave holding — what it is, what happens next, and what would make you call us sooner.
If you leave without understanding the plan, the visit is not finished. Ask again. Ask us to write it down. That is a normal request and a good one.
You are welcome to bring someone with you. For a long appointment, a difficult conversation, or if English is not your first language, a second person hearing the same information is genuinely useful.
We are in network with AHCCCS plans (Mercy Care, Health Choice, UnitedHealthcare Community Plan, Arizona Complete Health, Banner – University Family Care and CMDP), Aetna, Ambetter, Blue Cross Blue Shield, Cigna, UnitedHealthcare, Medicare, TRICARE West and others. Plan names change, so call (480) 745-3702 with the name of your plan before you book and we will tell you what to expect.
Copays, deductibles and coinsurance are due at the time of service. If you have no insurance, or choose not to use it, the visit is self-pay and you can ask for a Good Faith Estimate of the expected charges when you schedule. If you cannot pay a balance in full, call before it becomes past due and we will work with you on a payment plan.

Ask at your visit where we can. A refill decided in the room is faster than one requested later, and it means someone who has just seen you is deciding it.
Otherwise, ask your pharmacy to send the request. That reaches us in a form we can act on and avoids the transcription errors a phone message introduces.
Give it a few working days, and do not wait until the last tablet. For anything you cannot safely run out of — insulin, an inhaler, a heart or blood pressure medicine, an anticonvulsant — request it with a week to spare.
To get your records, or to have them sent somewhere, ask us — there is a form and we will tell you what it needs.
To bring records in, the fastest route is usually your previous practice sending them directly. Bring what you have on paper too; a discharge summary or a specialist letter in a bag is often more use than a file that has not arrived yet.
Ask for these at a visit rather than as a separate errand, and ask in June rather than late July if it is for the school year — our back-to-school page explains why that fortnight is the busiest of the year.
